Medication Administration

Information and Procedure 

In order for the district to administer medication to students, parents/guardians must complete and submit the appropriate form(s). Board of Education policy prohibits the distribution of medication to students unless the appropriate documentation is on file with the school.

  • Prescription Medications must have a filled out Medication Administration Order Form (see below), OR a written physician's order from the provider. All prescription medication forms must be signed by the provider and a parent/guardian. Forms are only valid for one school year. All prescription medication must be in the original, pharmacy labeled container. The pharmacy label must match the form/order for administration.
  • Non-prescription (over the counter) medications must have a Medication Administration Order Form completely filled out, and signed by a parent/guardian. Forms are only valid for one school year. Non-prescription (over the counter) medications must be in the original, unopened packaging, and must be labeled with the student's name and date of birth. 
  • ALL MEDICATIONS must be dropped off and picked up at your student's building main office/clinic by a parent/guardian. Students are not permitted to drop off or take home medications.
