Medication Administration
In order for the district to administer medication to students while students are at school, parents/guardians need to complete and submit the appropriate form(s) [available below] to school district medical personnel. Board of Education policy prohibits the distribution of medication to students on school property unless the appropriate documentation is on file with the school.

Select the form(s) for the necessary medication(s) from the list below. Forms are required for Tylenol and Motrin as well.

Complete the form(s). Forms should be signed by both a physician and a parent/guardian.

Parent/Guardian should drop off medication(s) and the appropriate completed form(s) (see below) in original packaging and accompanying form(s) at the security vestibule of the school that your child attends.
